Reporting and Surveys

Fall Site Visit forms will be emailed to ASPIRE Coordinators annually.

Fall Site Visit forms: DUE: At least 1 week prior to your scheduled Fall Site Visit.

The first step in reporting is the Fall Site Visit form. The information you provide in this form will include general details about your ASPIRE site, your program goals, and needs for the new academic year. This will provide a strong base for our Site Visit conversation. The form is tailored to you allowing you choose the type of ASPIRE site you are (MS, HS, MS/HS, CBO, online) and the model of ASPIRE program (mentoring and/or career and college readiness) you have.

End of year reporting is an opportunity for you to evaluate and review your site ASPIRE program goals and celebrate the work you have accomplished. The end of year report will take between 15 and 30 minutes to complete. Please take time to thoroughly answer all the questions.

This survey will include three sections Please have your site data ready to include.

Note: The form cannot be saved but you can leave the form and return to it as long as you keep the form open and don’t clear your cache.

Mentor/Hybrid ASPIRE Programs serving High School Students

Gather the following mentor program data point information prior to starting your End of Year Report Form.

ASPIRE HS Coordinators must provide an annual list of all graduating students (seniors/early grads) who were matched and met regularly with a community and/or staff mentor (meeting at least 3X). List must include:
1. Student’s full name
2. Student’s birthdate

Surveys are the primary way that ASPIRE gets feedback. Please encourage all community volunteers and graduating senior students to complete the ASPIRE surveys.
